Every year parents are required to update their student’s medication authorization for school. This authorization is for prescription and over the counter medications as well. The form needs to be turned in with your student’s medication on or before the first day of school.  All medications should be in their original packaging. You may bring the authorization form in to the school office or the doctor may fax it to us at 425-366-2602.
